Single Member
$89 MonthlyMember + Spouse or Child *
$159 Monthly * Child must be under 25 years oldFamily *
$229 Monthly * Family constitutes members + spouse and children up to 5 individuals. Additional family members $25/month.No long term contracts, cancel any time. There is a $150 enrollment fee per household

Dr. O' Boyle
Dr. Jeff O'Boyle completed postgraduate training in family medicine at Beaumont Hospital in Wayne, Michigan. He is board certified in family medicine by the American board of Family Medicine (ABFM) and American Osteopathic Board of Family Medicine (AOBFM) as well as board certified in Addiction medicine by the American Board of Preventive Medicine (ABPM).
Future of Coldwater Direct Primary Care
In June of 2026, the old 'Vested' building at 62 W. Chicago Ave in downtown Coldwater was acquired. Dr. O'Boyle is incredibly grateful to the prior owners for the great building and is excited to be a part of the future fabric of downtown Coldwater. The building will need a little care and investment before re-opening, so in the meantime Coldwater Direct Primary Care is leasing space at the Coach Eby Youth & Family Center at 89 W Chicago St, Coldwater. Stay tuned for exciting changes.
